Purchase Order Agreement
By signing this order, I hereby agree to all the terms and conditions as stated in our 'Standard Terms and Conditions' as well as all policies associated with 'Returns & Cancellations' in addition to the specified 'Lead Times'. The prices, specifications, and conditions are satisfactory and are accepted. The lead times, shipping delays, sold to, ship to, and bill to information are also accepted. I am authorized to place this order with full understanding of these terms.
1. TAX EXEMPTION: If applicable, a copy of the tax exemption certificate is required. 2. PURCHASE ORDER: A copy of the signed purchase order is required. 3. LEAD TIMES: Lead times can be inquired about via email@example.com, but is subject to change. Lead times are specifically listed on each product page at the time of ordering. These lead times are estimates. Once your order ships tracking information will be sent out via your email address associated with the order. 4. BUYERS RESPONSIBILITY: It is the buyers responsibility to understand the quality, warranty, return policy, features, options, appearance, and size of equipment purchased, prior to purchase. 5. DELIVERY and ASSEMBLY: Delivery and assembly are to be performed during regular business hours. Special requests necessary to complete work, will be paid for by customer. 6. CLAIMS: Customer agrees to promptly file claim for all goods damaged goods, regardless of damage or reason for claim. US Fitness Supply reserves the right to honor claims on a case-by-case basis.
